Mental Health Practitioner for Older Adults

Job Overview:
We are seeking a dedicated and compassionate Mental Health Practitioner to join our team at Derbyshire Healthcare NHS Foundation Trust. As an Older Adult Mental Health Practitioner, you will play a vital role in supporting individuals with functional mental health conditions in a community setting.
Main Duties:
- Conduct comprehensive person-centred psychosocial assessments, risk assessments, and formulation of presenting mental health needs.
- Build therapeutic relationships, develop a plan of care, and empower individuals to take control over their health.
- Support and motivate individuals to improve their mental health by helping them develop self-care skills, build resilience, and better utilise community resources.
- Maintain relationships with community partners, including statutory organisations, Voluntary, Community and Social Enterprise (VCSE) organisations, and the local community.
- Develop and deliver a flexible, responsive local service, taking forward an Integrated Wellbeing Pathway in the designated locality.
Requirements:
- Degree in Mental Health Nursing or Occupational Therapy.
- Post-qualification experience in the relevant field, with a focus on working with older adults with functional mental health diagnoses.
- Experience of community working and linking with partner agencies.
