About the Role

Care Outlook LTD is seeking a highly skilled and dedicated Care Coordinator to join our team in our Greenwich branch. As a Care Coordinator, you will play a vital role in maintaining and developing our domiciliary service, ensuring the highest quality of care for our service users.

Key Responsibilities
  • Roster and Template Care Calls: Utilize electronic systems to manage care calls and ensure efficient service delivery.
  • Direct Management of the Care Team: Supervise, appraise, and train staff to deliver exceptional care services.
  • Care Planning and Risk Management: Create, manage, and monitor customer care plans, ensuring staff rotas meet service users' needs and aspirations.
  • Quality Assurance and Compliance: Ensure care assessments, care, and risk planning meet best practice and customers' aspirations, while maintaining compliance with CQC regulations.
  • Staffing and Recruitment: Work with the Recruitment Team to ensure sufficient current and future staffing levels.
  • Emergency Response and Support: Provide care services to service users in emergencies, arrange emergency cover when required, and cover all reasonable calls.
  • Customer Service and Communication: Ensure telephones are answered promptly, and people are spoken to in a polite and respectful manner.
  • Quality Standards and Compliance: Work alongside the Branch Manager to ensure all quality standards, complaints, and comments are dealt with promptly.
  • On-Call Rota and Professional Development: Participate in the paid on-call rota and engage in ongoing support and professional development opportunities.

Requirements
  • Commitment to Quality Care: 100% committed to helping improve the quality of life of vulnerable people.
  • Flexibility and Availability: Flexibility to cover on-call to meet the needs of the business.
  • CQC Standards and Compliance: Knowledge of CQC standards and CQC compliance.
  • Experience in Care Coordinating: Experience with rostering care calls on an electronic system and care coordinating or 2 years working within health and social care.
  • Full Driving Licence and Vehicle Access: Full driving licence and access to a vehicle (preferred).
  • NVQ Level 3 Health and Social Care: NVQ Level 3 Health and Social Care (preferred).
