We are currently seeking a Care Coordinator to join our team in our Greenwich branch, where you will play a vital role in maintaining and developing our domiciliary service.
Key Responsibilities:
- Manage and coordinate care calls using an electronic system, ensuring efficient and effective service delivery.
- Directly supervise and develop the care team, providing regular feedback and coaching to ensure they meet the highest standards of care.
- Create and manage customer care plans, ensuring that staff rotas meet the needs and aspirations of our service users.
- Ensure that care assessments, care, and risk planning meet best practice and customer aspirations, promoting a culture of excellence and continuous improvement.
- Work closely with the Recruitment Team to ensure sufficient current and future staffing levels, ensuring that our service users receive the best possible care.
- Provide emergency care services to service users, arrange emergency cover when required, and cover all reasonable calls as needed.
- Ensure that telephones are answered promptly, and people are spoken to in a polite and respectful manner, reflecting the values of our organization.
- Collaborate with the Branch Manager to ensure that all quality standards, complaints, and comments are dealt with promptly and efficiently.
- Participate in the paid on-call rota, demonstrating flexibility and commitment to our service users.
- Implement measures to comply with CQC regulations and work towards achieving the best care standards possible, upholding the reputation of our organization.

We are looking for a Care Coordinator who is:
- Committed to improving the quality of life of vulnerable people.
- Flexible and able to cover on-call shifts to meet the needs of our service users.
- Familiar with CQC standards and CQC compliance, ensuring that our service meets the highest standards.
- Experienced in rostering care calls on an electronic system.
- Experienced in care coordinating or has 2 years' experience working within Health & Social Care, with a passion for delivering high-quality care.
- Hold a full driving license and have access to a vehicle, although this is not essential.
- Hold an NVQ Level 3 in Health & Social Care, although this is not essential.
