About the Role

We are seeking a skilled Care Specialist to join our team, providing support to vulnerable adults across Hertfordshire. This is a unique opportunity to make a meaningful impact on people's lives by being part of our Telecare Response Service.

Key Responsibilities
* Respond to calls from service users and provide practical and emotional support
* Conduct welfare checks and visits to ensure the well-being of our clients
* Work closely with clinicians and community health professionals to assess and meet individual needs
* Develop and implement personalized support plans for each service user

What You'll Need
* A full UK manual driving licence and access to a vehicle
* Good knowledge of services provided by the NHS and Social Care
* Excellent communication and interpersonal skills
* Ability to work independently and as part of a team

* Competitive salary: £12.00 per hour
* 36 days annual leave (including bank holidays) + option to buy 5 extra days

A Day in the Life
