Main Responsibilities- Conduct clinical assessments of patients and develop individualized treatment plans.
- Participate in the computerized dosing of patients and liaise with medical staff and nursing colleagues as needed.
- Provide education and support to patients and their families on anticoagulation management.
- Collaborate with the multidisciplinary team to ensure seamless care delivery.
