Job Title: Clinical, Education and Training Pharmacist

Mottingham, Downham & Chislehurst Primary Care Network (MDC PCN) is seeking an experienced and forward-thinking qualified pharmacist to join their pharmacy team.

Main Responsibilities:
  1. Optimise medicines within the PCN to improve patient outcomes.
  2. Provide expert advice on clinical medicines to healthcare professionals.
  3. Conduct structured clinical reviews with patients to manage complex polypharmacy needs.
  4. Work as part of a Multi-Disciplinary Team (MDT) to clinically assess and treat patients.
  5. Provide professional advice and support on safe prescribing, handling, and administration of medicines.
  6. Lead on medicines aspects relating to patient care, optimising medicines, reducing medicine-related risks, and improving adherence to taking medicines.
  7. Resolve day-to-day medicines and prescription issues with healthcare providers.
  8. Help patients manage their conditions and lead prescription services.
  9. Present changes to prescribing in line with NICE, best practice, or in response to prescribing alerts.
  10. Rationalise repeat prescription lists to avoid waste and duplication.
  11. Provide reviews to patients with long-term conditions or complex health needs, including drug monitoring and risk vs. benefit.
  12. Contribute to the management of the prescribing budget through safe, cost-effective rational prescribing.
  13. Minimise the harms associated with the use of medicines.
Person Specification:
  • Registered with the General Pharmaceutical Council.
  • Postgraduate qualification in clinical/community pharmacy or equivalent additional experience.
  • Evidence of recent continuing learning and professional development.
  • Diploma in clinical/community pharmacy.
Requirements:
  • Experience of working in multi-disciplinary teams.
  • Experience of medication reviews.
  • Experience of working within Primary Care.
  • Experience of Emis GP Computer System.
  • Knowledge of the changing NHS environment and structures.
  • Familiarity with analysing and interpreting ePACT data.
Skills and Abilities:
  • Excellent interpersonal skills.
  • Ability to work on own initiative, plan, prioritise, make decisions, and meet deadlines.
  • Ability to work well within a team.
  • Good organisational skills.
  • Good time management.
  • Ability to deal with difficult situations with diplomacy and sensitivity.
  • Confident and assertive in dealing with other healthcare professionals.
