About the Role

We are seeking a highly motivated Registered Mental Health Nurse or Allied Health Professional to join our All Age Continuing Care and Specialist Placements Team (AACC) at the Compass Centre in Chatham, Kent.

The successful candidate will be employed by NHS Kent and Medway Integrated Care Board (KMICB) and work primarily with patients originating from North Kent, placed in various geographical areas in England.

The team provides the total service delivery for all placements/care commissioned by the NHS Kent & Medway Integrated Care Board (KMICB) according to National Framework for NHS Continuing Healthcare and Funded Nursing Care (CHC/FNC).

The post holder will support the Associate Director and Assistant Head of AACC to manage Band 6 nurses and ensure correct processes are followed in the delivery of the National Framework for NHS Continuing Healthcare & NHS Funded Nursing Care.

Main Responsibilities

  • Undertake holistic assessments according to National Framework for NHS Continuing Healthcare and Funded Nursing Care.
  • Manage complex clinical cases, quality reviews, developing personalized care and support plans in the delivery of Personal Health Budgets (PHBs).
  • Assist the Associate Director and Assistant Head of AACC in responding to enquires or requests for information from political and legal bodies.
  • Manage CHC appeals process by preparing cases and clinical information for appeals at internal and external review panels.
