You will be responsible for assessing, planning, implementing, and evaluating patient care, using your clinical expertise and judgment to make decisions that promote the best possible outcomes for patients.
Key Responsibilities- Assess patients' needs and develop individualized care plans
- Provide high-quality patient care, including wound management, palliative care, and rehabilitation
- Work collaboratively with other healthcare professionals to ensure seamless care coordination
- Participate in multidisciplinary team meetings and contribute to the development of care plans
- Maintain accurate and up-to-date patient records
- Diploma or degree in Nursing
- Current NMC registration
- ENB 998/mentorship preparation or equivalent
- Experience of working in a community setting
- Ability to work autonomously and as part of a team
- Excellent communication and interpersonal skills
