Senior Pharmacist

University Hospitals Coventry and Warwickshire NHS Trust is seeking a highly skilled Senior Pharmacist to join our team in the ASCVD/AF Clinics. As a key member of our clinical pharmacy team, you will play a vital role in improving the quality of care for patients treated for atrial fibrillation (AF) and atherosclerotic cardiovascular disease (ASCVD) within the Coventry and Warwickshire ICB.
About the RoleThe successful candidate will be responsible for providing a cost-effective clinical pharmacy service, reviewing patients within primary care who are not optimised for the above conditions, and working closely with secondary care colleagues to identify patients who are not optimised on their lipid lowering therapy in line with local guidance.
You will also be tasked with reviewing patients who have a diagnosis of non-valvular atrial fibrillation (NVAF) in primary care and optimising their anticoagulation regimen if appropriate. Additionally, you will be responsible for data collection to demonstrate benefits to patient population in completing these interventions, as well as working closely with other healthcare professionals to provide teaching and education sessions.
Key Responsibilities- Identification of sub-optimally treated ASCVD patients who are not achieving target lipid levels as outlined in local guidelines
- Identification of patients with NVAF who require anticoagulation, and initiating therapy where appropriate
- Review of treatment options for lipid management and anticoagulation in appropriate patient populations, in collaboration with patients
- Counsel and optimise cardiovascular medications in patients where appropriate
- Identification of ASCVD patients, following risk assessment review, of ASCVD patients who are sub-optimally treated for lipid management
