Senior Pharmacist

We are seeking a highly skilled and experienced Senior Pharmacist to join our team at University Hospitals Coventry and Warwickshire NHS Trust. The successful candidate will be responsible for providing a cost-effective clinical pharmacy service to patients treated for atrial fibrillation (AF) and atherosclerotic cardiovascular disease (ASCVD) within the Coventry and Warwickshire ICB.
The ideal candidate will have a strong background in clinical pharmacy and experience in lipid management and anticoagulation therapy. They will be expected to work closely with secondary care colleagues to identify patients who are not optimised for the above conditions and provide education and training to healthcare professionals.
The role will involve data collection to demonstrate benefits to patient population in completing these interventions, as well as working closely with other health care professionals to provide teaching and education sessions.
Key Responsibilities:- Identification of sub-optimally treated ASCVD patients who are not achieving target lipid levels as outlined in local guidelines
- Identification of patients with NVAF who require anticoagulation, and initiating therapy where appropriate
- Review of treatment options for lipid management and anticoagulation in appropriate patient populations, in collaboration with patients
- Counsel and optimise cardiovascular medications in patients where appropriate
- Identification of ASCVD patients, following risk assessment review, of ASCVD patients who are sub-optimally treated for lipid management
