Clinical Pharmacist - Medicines Optimisation Specialist

**Job Summary**

We are seeking a Clinical Pharmacist to join our Primary Care Network team in Coventry. The successful candidate will work as part of a multi-disciplinary team to develop and support medicines optimisation work across the 11 GP practices within the network.

**Key Responsibilities**

* Work as part of a multi-disciplinary team in a patient-facing role
* Support delivery of the new network Directly Enhanced Service (DES)
* Conduct structured medication reviews to proactively manage patients with complex polypharmacy
* Provide clinical medicines expertise to both patients and practice teams
* Improve prescribing practices across the PCN
* Improve service and quality through mechanisms such as clinical audits
* Interface with community and hospital colleagues to help resolve medication-related issues

**Requirements**

* Registered with the General Pharmaceutical Council
* Postgraduate diploma/MSc in Clinical or Community Pharmacy or equivalent
* Non-medical prescriber registration
* Advanced assessment and history taking skills

**Person Specification**

* Vocational 4-year Masters degree in pharmacy
* 1 year pre-registration training
* Evidence of Continuing Professional Development
* Good critical appraisal skills
* Experience of patient consultation
* Experience of working with GPs
* Post-registration experience in primary, secondary or community pharmacy
* Experience of clinical pharmacy

**Knowledge**

* Good understanding of primary care structure and General Practice
* Good clinical knowledge
* Able to apply clinical knowledge appropriately in a variety of practice settings or patient groups

**Disclosure and Barring Service Check**

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

**About the Role**

The successful candidate will join a core PCN Pharmacy Team and will be supported by a Senior Clinical Pharmacist and a GP Supervisor. Regular development sessions, teaching, training and opportunities to upskill will be provided.
