### Job Description

#### Role Summary

We are seeking a highly skilled Clinical Pharmacist to join our team in a patient-facing role, working as part of a multi-disciplinary team in primary care. The successful candidate will be responsible for developing and supporting medicines optimisation work across the 11 GP practices within the Primary Care Network (PCN).

#### Key Responsibilities

* Conduct structured medication reviews to proactively manage patients with complex polypharmacy
* Provide clinical medicines expertise to both patients and practice teams
* Improve prescribing practices across the PCN
* Improve service and quality through mechanisms such as clinical audits
* Interface with community and hospital colleagues to help resolve medication-related issues

#### Person Specification

* Registered with the General Pharmaceutical Council
* Postgraduate diploma/MSc in Clinical or Community Pharmacy or equivalent
* Non-medical prescriber registration
* Advanced assessment and history taking skills
* Good critical appraisal skills
* Experience of patient consultation
* Experience of working with GPs
* Post-registration experience in primary, secondary, or community pharmacy

#### Working Arrangements

* The post-holder will work as part of a multi-disciplinary team in a patient-facing role
* The successful candidate will join a core PCN Pharmacy Team and will be supported by a Senior Clinical Pharmacist and a GP Supervisor
* Regular development sessions, teaching, training, and opportunities to upskill will be provided

#### Conditions of Employment

* The post-holder will be required to comply with the organisation's policies and practices
* The post-holder will be required to keep up to date with IG And GDPR legislation
* The post-holder will be required to ensure knowledge of Safeguarding policies and training is up to date
