About the Role

We are seeking a highly motivated and empathetic individual to join our team as a Post Diagnostic Support Link Worker in the Highland locality. As a key member of our Community Mental Health Team, you will play a vital role in providing person-centred support to individuals with dementia and their families.

Key Responsibilities
  • Provide high-quality, person-centred support to individuals with dementia and their families, focusing on enabling them to live well with the condition.
  • Work collaboratively with colleagues within Alzheimer Scotland, the NHS, and other stakeholders to build and maintain relationships and deliver effective support.
  • Utilize digital platforms to facilitate virtual meetings and online groups, ensuring a blended approach to support.
  • Develop and maintain a deep understanding of dementia and its impact on individuals and their families, as well as excellent communication skills and a warm, flexible approach.
  • Hold a recognized relevant professional qualification and have skills and knowledge at the Enhanced Level of the Promoting Excellence Framework or be working towards its completion.
  • Possess a full driving license and have access to a car.
