If so, we invite you to join our team of Post-Diagnostic Support Link Workers (PDS Link Workers) at NHS Scotland. We provide personalized support to people diagnosed with dementia and their families for at least a year following diagnosis, aligning with the Scottish Government's minimum guarantee.

Our Mission: Enabling Well-being with Dementia

The PDS National Service aims to empower individuals to live fulfilling lives with dementia. Our approach is centered around Alzheimer Scotland's 5-pillar model, guiding individuals towards self-management and independence.

Key Pillars of Support:

  • Understanding the illness and adjusting to the diagnosis
  • Peer support networks
  • Maintaining community connections
  • Planning for future care and decision-making
  • Developing outcome-focused plans aligned with individual aspirations

Collaboration is Key:

As a PDS Link Worker, you will collaborate effectively within our team, building strong relationships with colleagues from Alzheimer Scotland, the NHS, and other stakeholders. Your role requires proficiency in digital platforms as we utilize a blended approach to support, incorporating virtual meetings and online groups.

Essential Qualifications and Skills:

  • Recognized relevant professional qualification
  • Comprehensive understanding of dementia and its impact on individuals and families
  • Exceptional communication skills, warmth, flexibility, empathy, and relationship-building abilities
  • Skills and knowledge at the Enhanced Level of the Promoting Excellence Framework or demonstrable progress towards completion
  • Full driving license and access to a car
