We are seeking a passionate and dedicated Care Coordinator to join our team at The Disabilities Trust, working in partnership with individuals living with acquired brain injury, autism, and learning or physical disabilities. Job Summary

This role involves leading a team of nurses and collaborating with an interdisciplinary team to deliver high-quality, personalised care and support. As a Care Coordinator, you will be responsible for developing and implementing individualised care plans, ensuring seamless transitions between services, and facilitating communication between healthcare professionals, families, and carers.

About the Role

We are looking for an experienced nurse who is committed to delivering exceptional patient outcomes and is passionate about making a difference in the lives of our service users. As a key member of our team, you will contribute to the development of innovative service improvement initiatives, participate in training and education programs, and provide mentorship and guidance to colleagues.

What We Offer
  • An estimated salary range of £45,195 - £50,395 per annum, depending on experience.
