About the Role
Priory Medical Group is seeking a skilled Digital and Care Continuity Coordinator to join our dynamic team. As a key member of our digital team, you will play a vital role in ensuring our non-digital patients are not excluded from care co-ordination.
Main Responsibilities
As a Digital and Care Continuity Coordinator, you will be responsible for:
- Coordinating patient care to ensure our non-digital patients are not excluded from care co-ordination.
- Using non-digital communication tools to contact patients, process patient responses, and maintain our digital systems and tools.
- Working with the rota, digital and patient central team to confirm clinic capacity available for urgent care.
- Coordinating care for patients, including the most vulnerable and complex.
