Community Care Practitioner Role

We are looking for a skilled Community Care Practitioner to join our team in Maidstone. As a care support worker, you will work on a rota basis providing care and support 24/7, with day shifts ranging from 7am to 10pm and sleep-in shifts starting at 10pm.

About the Role

  • Deliver Person-Centred Care to service users
  • Develop Relationships with service users, families, and other professionals
  • Maintain Accurate Records of care and support provided
