Nurse Practitioner for Community Care

As a Nurse Practitioner for Community Care, you will play a vital role in supporting our Virtual Ward model, collaborating with the acute trust to provide hospital-standard treatment within patients' homes.
The Virtual Ward philosophy focuses on leading care by specialists, providing acute clinical care at home for short durations of up to 14 days. This community service is nurse-led, with access to support at the Virtual Monitoring Hub. Our goal is to prevent unnecessary hospital admissions and facilitate early discharge from acute hospitals, ensuring seamless transitions for patients.
This role is an excellent opportunity to work in a dynamic environment, utilizing your skills and experience to make a significant impact on patient outcomes. You will be part of a multidisciplinary team, working collaboratively to deliver high-quality care to patients in their own homes.
Key Responsibilities:
- Assessment, planning, implementation, and evaluation of holistic care for patients requiring hospital-level interventions at home.
- Development of integrated care plans in partnership with patients, ensuring person-centered approaches and addressing individual needs.
- Ordering equipment and referring patients as necessary to ensure continuity of care.
- Supporting the supervision and development of junior staff, including Healthcare Assistants and Students.
- Active participation in the multi-professional team to provide high-quality care to patients, promoting effective communication and collaboration.
- Undertaking competent clinical interventions and identifying any deterioration in patients' conditions, taking appropriate action to ensure timely interventions.
- Managing a caseload of patients with complex needs, associated with specific specialties, and providing tailored care solutions.
- Supporting duty roles and completing triages to ensure seamless coverage and efficient allocation of resources.

Salary Information
The estimated salary range for this role is £38,890 - £44,442 per annum, depending on experience and qualifications. Please note that this information is provided for guidance purposes only and may be subject to change.
