Job Description:
We are seeking a skilled Health and Wellbeing Practitioner to join our team at The Forward Trust.

Key Responsibilities:
• Carry out assessments to inform treatment journeys
• Deliver structured 1:1 and group-work sessions for substance misuse and low-level mental health service users
• Facilitate substance misuse and mental health groups and wellbeing and recovery programmes
• Support service users in developing release plans

Requirements:
• Experience working in a Health or Justice setting
• Knowledge of substance misuse and low-level mental health issues
• Ability to design and implement SMART care plans
• Familiarity with psychosocial interventions and motivational interviewing techniques
