As an Independence Support Worker, you will play a pivotal role in empowering individuals to regain independence after hospital discharge.

Working closely with occupational therapists, you will co-develop personalised reablement plans, focusing on customer goals and outcomes.

The estimated salary for this position is £23,893 - £24,702 per annum, including 10% unsocial hours for qualifying shifts.

The successful candidate will demonstrate flexibility, adaptability, and excellent communication and interpersonal skills, with a genuine passion for helping others.
