**Job Description:**

  • Carry out comprehensive assessments to determine the needs and support requirements of individuals with learning disabilities.
  • Create and implement individualized support plans to enhance independence and overall well-being.
  • Collaborate with various stakeholders, including healthcare professionals, educators, and housing specialists, to ensure a cohesive approach.
  • Offer guidance and support to individuals and their families.
  • Maintain accurate and up-to-date records of cases.

**Requirements:**

  • Bachelor's degree in Social Work
  • Registration with Social Work England
  • Experience working with adults with learning disabilities
  • Familiarity with relevant laws and guidelines
  • Outstanding communication and interpersonal skills
  • Able to work effectively under pressure and meet deadlines

The estimated annual salary for this position is approximately £61,200, based on a 30-hour workweek and £30 per hour pay rate. This is a 6-month contract opportunity.
