Community Care Coordinator

As a **Case Worker**, you will be part of our team that provides support to individuals in need. You will work closely with our clients to understand their strengths and weaknesses, and develop personalized plans to help them overcome their challenges.
The role requires a driving licence and access to a vehicle, as you will be required to visit clients in various locations. In return, we offer a competitive salary of £28,000 - £30,000 per annum, depending on experience.
**Key Responsibilities:**
- Provide support to individuals experiencing mental health challenges
- Develop and implement personalized plans to help clients achieve their goals
- Work closely with clients to understand their strengths and weaknesses
- Maintain accurate records and reports
