Job Overview

We are seeking an experienced dietitian to join our dynamic Community Dietetics Team in Oldham. As an autonomous practitioner, you will work in various settings, including community clinics, care and nursing homes, and patients' own homes.

Your key responsibilities will include ensuring seamless discharge of patients from acute to community settings, supporting patients discharged before being seen by acute dietitians and A&E, and liaising with the Community Dietetics team, local GPs, care agencies, and Urgent Care Team to manage complex patients.

You will work with multidisciplinary teams to reduce attendance at A&E, improve independence, and advise on the appropriate use of oral nutritional supplements. You will also provide supervision for staff within the community dietetic team and contribute to student training.

Key Responsibilities
  • Work as an autonomous practitioner in various community settings
  • Ensure seamless discharge of patients from acute to community settings
  • Liaise with the Community Dietetics team, local GPs, care agencies, and Urgent Care Team
  • Provide supervision for staff within the community dietetic team
  • Contribute to student training
Requirements
  • Registered as a Dietitian with the HCPC
  • Membership of the BDA specialist group
  • Relevant clinical experience of enteral feeding and oral nutrition support
  • NHS experience as a senior dietitian
  • Experience/exposure to management of HEF caseload
  • Evidence of enhanced skills in a specialist area within dietetics
  • Working/experience in acute setting
  • Working in/liaison with community dietitians/care homes
