NHS Chadderton Medical Practice is seeking a highly skilled Senior Advanced Practice Nurse Lead to join our team. As a key member of our primary care services, you will play a vital role in delivering exceptional patient-centred care.
**Job Summary**
We are looking for an experienced and ambitious Advanced Practice Nurse with strong leadership skills to lead our team in providing high-quality healthcare services to our patients. The successful candidate will be responsible for managing acute and chronic medical conditions, developing and implementing health promotion programs, and working closely with patients to achieve optimal health outcomes.
**Key Responsibilities**
- Develop and lead health promotion programs to improve patient wellbeing and health literacy
- Manage acute and chronic medical conditions through effective treatment planning and evaluation
- Provide advanced nursing care, wound care, and specialist clinics (e.g., contraception, ear syringing, travel medicine)
- Request and interpret pathology tests, maintaining accurate clinical records
- Prescribe medications, deliver immunisation programs, and manage mental health needs
**Requirements**
To be successful in this role, you will need:
- A Master's degree in a relevant field and postgraduate diploma or degree in Advanced Practice Qualification
- Registration with the Nursing and Midwifery Council as an Independent Nurse Prescriber
- Experience of working in a primary care setting, preferably in a similar role
**What We Offer**
