Job Description

The Advanced Nurse Practitioner (ANP) at Chadderton Medical Practice plays a vital role in delivering high-quality, patient-centred care. As a highly skilled and experienced healthcare professional, the ANP has demonstrated advanced clinical competency and expertise in diagnosing, treating, and managing a broad range of health conditions.

Main Responsibilities
  1. Develop and lead health promotion and wellbeing programmes.
  2. Manage acute and chronic medical conditions, creating and evaluating treatment plans.
  3. Provide advanced nursing care, wound care, and specialist clinics (e.g., contraception, ear syringing, travel medicine).
  4. Request and interpret pathology tests, and maintain accurate clinical records.
  5. Prescribe medications, deliver immunisation programmes, and manage mental health needs.

  1. Develop, implement and embed health promotion and wellbeing programmes.
  2. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice.
  3. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients' health.
  4. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.
  5. Request pathology services as necessary.
  6. Process and interpret pathology and other test results as required.
  7. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
  8. Maintain accurate clinical records.
  9. Prioritise health issues and intervene appropriately.
  10. Recognise, assess and refer patients presenting with mental health needs.
  11. Implement vaccination programmes for adults and children.
  12. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required.
  13. Support the clinical team with all safeguarding matters in accordance with local and national policies.
  14. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.
  15. Deliver opportunistic health promotion where appropriate.
  16. Provide supervision and support to other members of staff such as Healthcare Professionals, Practice Nurses, Nurse Associates and Nurse Practitioners.
Person Specification

The ideal candidate will have:

  • Registered Nurse with Nursing and Midwifery Council.
  • Master's Degree required for qualification post.
  • Post Graduate diploma or degree for Advanced Practice Qualification.
  • Qualified Independent Nurse Prescriber on the NMC Register.
  • Meets NMC re-validation requirements in accordance with the NMC Re-validation booklet.
  • Experience of working in Primary Care setting.
