About the Role

This is an exciting opportunity to join NHS Professionals Limited as a Community Health Nurse, working in a dynamic and supportive environment. As a Community Health Nurse, you will be responsible for providing high-quality patient care in a community setting, working closely with patients, families, and healthcare teams to promote health and well-being.

Key Responsibilities

  • Assess and monitor patient conditions, arranging diagnostic tests as needed
  • Recognize and respond to changes in patient condition, taking appropriate action to escalate concerns
  • Manage emergency situations, demonstrating excellent analytical skills and sound decision-making
  • Develop and implement individualized care plans, promoting patient autonomy and independence
  • Collaborate with multidisciplinary teams to coordinate patient care, ensuring seamless transitions and optimal outcomes

Requirements

  • Registered Nurse with current NMC registration
  • Minimum 6 months paid experience in a community environment within the last 2 years
  • Valid UK driving license and access to a car
