As a Pathway Advisor, you will be involved in planning and delivering a range of activities, opportunities, and services to young people aged 16 to 25 who have been in our care.

Key responsibilities include providing advice and support, drawing up the Pathway Plan, keeping in touch with the young person, and co-ordinating services, linking in with other agencies.

We are a service with high ambitions and aspirations for all our young people, and we are looking for someone with a background in working with adults who have been impacted by their emotional and mental health.   • Deliver a range of activities, opportunities, and services to young people aged 16 to 25 who have been in our care.
