Main Duties

As a Community Mental Health Practitioner, you will be responsible for:

  • Providing comprehensive mental health assessments and developing individualized care plans
  • Working closely with service users, families, and carers to promote recovery and well-being
  • Collaborating with other healthcare professionals to ensure seamless care coordination
  • Providing education and support to service users and their families on mental health issues and recovery strategies
  • Participating in multidisciplinary team meetings to discuss service user care and develop strategies for improvement

Person Specification

We are looking for an individual with:

  • Registered Mental Health Nurse Degree/Diploma
  • On-going registration with the NMC
  • Specialist qualification or equivalent level in skills and development
  • Experience of working in a community mental health setting
  • Ability to work as part of a multidisciplinary team
  • Excellent communication and interpersonal skills
