About the Role

Lancashire & South Cumbria NHS Foundation Trust is seeking an experienced Mental Health Practitioner to join our Community Mental Health Team.

As a Community Mental Health Nurse, you will play a vital role in providing high-quality care to individuals in the community, working closely with a multidisciplinary team to assess, plan, and implement care plans.

Key responsibilities include:

  • Providing nurse leadership within the multidisciplinary team
  • Undertaking mental health assessments, including risk assessments
  • Referring to other clinicians and services as necessary
  • Providing a fast and responsive service to assess individual needs

Requirements

To be successful in this role, you will need:

  • Registration with the NMC
  • Specialist qualification or equivalent level in skills and development
  • Experience of working as part of a multidisciplinary team
  • Ability to assess, plan, and implement care plans
