About the Role

Lancashire & South Cumbria NHS Foundation Trust is seeking an experienced Mental Health Practitioner to join our Community Mental Health Team.

As a Community Care Specialist, you will be responsible for providing high-quality care to individuals in the community, working closely with a multidisciplinary team to assess, plan, and implement care plans.

Main Responsibilities

  • Provide nurse leadership within the multidisciplinary team, ensuring effective assessment, formulation, planning, and monitoring of care given to service users.
  • Undertake mental health assessments, including risk assessments, within a multi-disciplinary assessment team.
  • Refer to other clinicians within the service or refer to other services/agencies where necessary.
  • Provide a fast, responsive service to assess individual's needs.
