About the Role

We are seeking an experienced and skilled Community Mental Health Nurse to join our team at Lancashire & South Cumbria NHS Foundation Trust. As a Community Mental Health Nurse, you will play a vital role in providing high-quality care to individuals with mental health needs in the community.

Key Responsibilities
  • Provide comprehensive mental health assessments, including risk assessments, to individuals in the community.
  • Develop and implement personalized care plans in collaboration with the multidisciplinary team.
  • Work closely with other healthcare professionals to ensure seamless care coordination and effective communication.
  • Provide education and support to individuals, families, and carers to promote mental health and well-being.
  • Contribute to the development and implementation of mental health services and policies within the trust.
