We are seeking a highly skilled and experienced Patient Care Navigator Team Leader to join our team at Tyntesfield Medical Group. As a key member of our operations team, you will be responsible for leading and managing the Patient Care Navigator team, ensuring the delivery of high-quality patient care and services.
About the RoleThe Patient Care Navigator Team Leader will be responsible for:
- Leading and managing the Patient Care Navigator team to ensure the delivery of high-quality patient care and services.
- Developing and implementing effective strategies to improve patient satisfaction and experience.
- Collaborating with the Operations Manager and other stakeholders to ensure seamless communication and coordination of patient care services.
- Providing guidance and support to Patient Care Navigators to ensure they have the necessary skills and knowledge to deliver high-quality patient care.
- Monitoring and evaluating the performance of the Patient Care Navigator team to identify areas for improvement and implement changes as needed.
To be successful in this role, you will need:
- Excellent leadership and management skills, with the ability to motivate and inspire a team.
- Strong communication and interpersonal skills, with the ability to build effective relationships with patients, staff, and stakeholders.
- Experience of working in a healthcare setting, with a strong understanding of patient care and services.
- Ability to work independently and as part of a team, with a flexible and adaptable approach to work.
- Good IT skills, with the ability to use software and hardware to support patient care services.
