About the Role

The Care Navigator is a critical position in our healthcare team, responsible for providing exceptional patient services and ensuring seamless access to our services. As a Care Navigator, you will work closely with our patients, healthcare professionals, and administrative staff to deliver high-quality care and support.

Key Responsibilities

  • Provide responsive and professional reception and patient navigation services to our patients.
  • Sign-post patients appropriately to practice services in accordance with our navigation Standard Operating Procedures.
  • Make appointments for patients attending or calling by telephone, using our computer system, and in accordance with our Practices appointment protocols.
  • Receive telephone requests and make arrangements for home visits.
  • Deal with all enquiries received by telephone or in person, as appropriate.
  • Work under the direction of other members of the team to provide communication services as requested and required.

Requirements

  • Good basic education to GCSE level or equivalent.
  • Experience of dealing with the public in a retail, commercial or public sector role.
  • Front line work in a very customer-focused industry.
  • IT-literate and with basic knowledge of web-based and Windows-based systems.
  • Able to multi-task and prioritise.
  • Excellent people skills, including dealing with challenging people.
  • Calm and well-organised under pressure.
  • Empathetic.
  • Honest.
  • Punctual.
  • Excellent communication skills.
