Job Title: Field Care Supervisor

We are seeking a highly skilled and experienced Field Care Supervisor to join our team at City and County Healthcare Group Ltd. As a Field Care Supervisor, you will play a vital role in supporting the delivery of high-quality care support services to our clients.

Key Responsibilities:
  • Support the planning and delivery of care support services in the community
  • Conduct regular reviews of individual services to ensure they meet the needs of our clients
  • Supervise and support care workers in the field to ensure they are providing high-quality care
  • Work collaboratively with care coordinators to plan realistic and effective care worker rotas
  • Participate in multi-disciplinary reviews of service users' needs
Requirements:
  • Experience in carrying out initial risk and needs assessments
  • Ability to work in partnership with service users, their families, carers, and healthcare professionals
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
