Job Title: Field Care Supervisor

We are seeking a highly skilled and experienced Field Care Supervisor to join our team at City and County Healthcare Group Ltd. As a Field Care Supervisor, you will play a crucial role in supporting the delivery of high-quality care support services to our clients.

Key Responsibilities:
  • Support the planning and delivery of care support services in the field, including the assessment and review of individual services.
  • Supervise and support care workers in the field, ensuring they have the necessary skills and knowledge to provide high-quality care.
  • Work with Care Coordinators to plan realistic and effective care worker rotas, taking into account the needs of individual clients and the skills and attributes of care workers.
  • Carry out regular reviews of individual services, including risk assessments and needs assessments.
  • Participate in multi-disciplinary reviews of service users' needs and provide input on care plans.
Requirements:
  • Experience in carrying out initial risk and needs assessments in respect of individual service users.
  • Ability to work in partnership with service users, their families, carers, advocates, healthcare professionals, and others to ensure seamless delivery of holistic packages of individual care support.
  • Strong communication and interpersonal skills, with the ability to build relationships with clients, care workers, and other stakeholders.
  • Ability to work independently and as part of a team, with a flexible approach to working hours and locations.
