Job Title: Field Care Supervisor

As a Field Care Supervisor at City & County Healthcare Group, you will play a vital role in delivering high-quality care support services to our clients. Your primary responsibility will be to support the delivery of care services by carrying out a range of functions in the field, including assessment and review of individual services, and practical supervision of care workers.

Key Responsibilities:
  • Support the planning of realistic and effective care worker rotas, taking into account the needs of individual clients and the skills and attributes of care workers.
  • Carry out regular reviews of individual services, including repeated and renewed needs and risk assessments.
  • Participate in multi-disciplinary reviews of service users' needs, working with healthcare professionals and other stakeholders to ensure seamless delivery of care.
  • Shadow new care workers in the field to ensure their competence and provide on-site observation and evaluation of care worker practice.
Requirements:
  • Experience in carrying out initial risk and needs assessments for individual service users.
  • Ability to work in partnership with service users, their families, carers, advocates, and healthcare professionals to deliver holistic packages of care.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
