About the Role:

We are seeking a Clinical Development Manager to join our team at NHS Sussex ICB. As a Senior Practitioner, you will be responsible for delivering a diverse range of functions across the Community Health Care (CHC) service.

Main Responsibilities:

  • Assessment Reviews and Evaluations
  • Care Planning and Development
  • Case Management and Coordination

This role involves working with colleagues to ensure safe, confident, and effective delivery of services across the whole care pathway.
