Nurse Care Coordinator

We are seeking an experienced Nurse to join our team as a Nurse Care Coordinator.
About the Role:
As a Nurse Care Coordinator at Barchester Healthcare, you will be responsible for delivering high-quality care to our residents. Your primary goal will be to ensure that each resident receives personalized attention and support to meet their unique needs.
In this role, you will work closely with our care teams to assess residents' physical, psychological, and social needs. You will develop and implement individualized care plans, taking into account their medical history, preferences, and goals.
A key aspect of this role is your ability to communicate effectively with residents, families, and healthcare professionals. You will provide emotional support, guidance, and education to help residents make informed decisions about their care.
To succeed in this position, you must have excellent clinical skills, a compassionate approach, and strong interpersonal abilities. You should be able to work independently and as part of a team, prioritizing tasks and managing time efficiently.
Responsibilities:
* Assess residents' needs and develop personalized care plans
* Coordinate care delivery with interdisciplinary teams
* Communicate effectively with residents, families, and healthcare professionals
* Provide emotional support and guidance to residents and families
* Collaborate with other departments to ensure seamless care transitions
* Maintain accurate records and reports
* Participate in quality improvement initiatives and training programs
Requirements:
* Current NMC registration
* Up-to-date clinical knowledge and skills
* Excellent communication and interpersonal skills
* Ability to work independently and as part of a team
* Strong problem-solving and critical thinking skills
* Compassionate and empathetic approach to care
