Job Overview

We are seeking a highly skilled and dedicated Mental Health Discharge Specialist to join our team. As a key member of our PARTNER Repatriation Service, you will play a crucial role in ensuring smooth and timely transitions for outpatients from out-of-county enhanced rehabilitation units into community-based services.

Main Responsibilities

Your key responsibilities will include:

  • Coordinating clinical and support services relevant to discharge planning.
  • Identifying the need for and coordinating MDT meetings/case conferences and supporting private care providers in the decision-making process regarding safe, effective, and timely discharge of patients.
  • Carrying a caseload and working collaboratively on the management of cases, ensuring liaison with community services to support timely discharge from locked rehabilitation/specialist placements.
  • Participating in all aspects of discharge and aftercare, including cooperation with Section 117.
  • Ensuring that risk assessments are carried out in conjunction with other agencies.
  • Attending commissioning panels acting as an advocate for patients.
  • Establishing and maintaining communication with individuals and groups about complex discharges, overcoming any problems in communication.
  • Involving, supporting, informing, and educating family/carers where appropriate throughout the discharge process.
Requirements

To be successful in this role, you will need:

  • A Social Worker qualification and significant relevant post-qualification experience.
  • Liaison with other agencies, social care, and the voluntary sector.
  • Experience of the funding processes for NHS-funded care and social care.
  • Discharge planning experience.
  • MDT working experience.
  • Experience of conducting and contributing to ongoing assessments.
