Job Summary

We are seeking a skilled Clinical Discharge Coordinator to join our Inpatient Pathway Team. The successful candidate will have experience working in adult and older adult mental health services and be able to utilize their clinical and risk assessment skills to ensure the best outcomes for patients as part of the admission and discharge process.

Main Responsibilities
  1. To work collaboratively with the ward multidisciplinary team, Social Services, and other relevant personnel to plan and manage timely and appropriate transfers of care for patients from the hospital.
  2. To work in partnership with wards, community teams, and external Health agencies to provide all aspects of care following discharge.
  3. Experience of teaching and training others.
  4. Experience of undertaking autonomous mental health assessments.

Requirements
  1. Knowledge of recent developments in the NHS and application of research-based practice, particularly in relation to patient flow and discharge planning.
  2. Knowledge and understanding of adult and child protection issues.
  3. Evidence of continued professional development and reflective practice.

Skills and Abilities
  1. Ability to communicate clearly and effectively in English in both verbal and written formats.

Additional Criteria
  1. Able to carry out the duties of the post, including being able to travel around the Trust area.
