We are seeking an experienced Care Coordinator to join our dynamic team at the Perinatal Mental Health Team, part of Herefordshire and Worcestershire Health and Care NHS Trust.

About the Role

The successful candidate will work as part of a multi-disciplinary clinical team, providing support to women at risk of developing or experiencing moderate to severe mental health difficulties during pregnancy and the postnatal period. The role involves completing assessments, supporting groups, and working alongside patients, their families, carers, and infants.

Key Responsibilities

  • Work in support of the close Multi-Disciplinary Team (MDT) within a community mental health setting
  • Supervise and teach junior staff as part of the role
  • Complete assessments and support with groups
  • Work alongside all members of the MDT, patients, their families, carers, and infants

About Us

The Perinatal Mental Health Team is a specialist community-based service focused on helping women at risk of developing or experiencing moderate to severe mental health difficulties during pregnancy and the 2 years following childbirth or require pre-conceptual counselling. We work with a number of partners, including GPs, Social Workers, Midwives, and Health Visitors, to ensure that patients' needs are met and families are supported.

Experience and Qualifications

  • Experience of care work within an appropriate community mental health setting
  • Case load management
  • Management of complex mental health needs in a community setting
  • Experience of counselling skills in a relevant area

Knowledge and Training

  • Knowledge of caring for people in the community
  • Knowledge of Person-centred Care
  • Knowledge of Mental Health Act 1983 and amendments
  • Knowledge of current mental health issues and legislation
  • Knowledge of risk assessment
