About the Role
We are seeking a highly skilled and dedicated Clinical Care Specialist to join our team at Lincolnshire Partnership NHS Foundation Trust. As a Clinical Care Specialist, you will be responsible for providing high-quality clinical care to individuals with learning disabilities and/or autism, ensuring safe and effective assessment, treatment, and comprehensive discharge planning.
Key Responsibilities
- Provide intensive and enhanced support to individuals with learning disabilities and/or autism, working closely with the Crisis Resolution and Home Treatment Service.
- Assess, plan, and implement evidence-based care to service users diagnosed with a Learning Disability and/or Autism who meet the criteria for the Crisis Resolution and Home Treatment service.
- Deliver care jointly alongside other members of the crisis team and the Transforming Care Liaison Team, providing specialist advice, guidance, and intervention in relation to an individual's learning disability/autism diagnosis.
- Operate flexibly across a 7-day week, working a rota system to provide this additional support.
Person Specification
We are looking for a qualified Registered Mental Health/Learning Disability Nurse or Social Work qualification, with active registration on the appropriate professional register. You will also have evidence of specialised continued professional training in clinical practice and relevant experience of working with adults and/or children/young people with a Learning Disability and/or autism.
