About the Role

We are seeking a highly skilled and experienced Clinical Care Specialist to join our team at Lincolnshire Partnership NHS Foundation Trust. As a Clinical Care Specialist, you will be responsible for providing high-quality clinical care to individuals with learning disabilities and autism, ensuring safe and effective assessment, treatment, and comprehensive discharge planning.

Key Responsibilities

  • Provide clinical care to individuals with learning disabilities and autism, including assessment, treatment, and discharge planning.
  • Work collaboratively with other members of the crisis team and the Transforming Care Liaison Team to provide intensive and enhanced support to individuals with complex needs.
  • Develop and implement individualized care plans, taking into account the unique needs and circumstances of each person.
  • Provide training and support to colleagues on best practices in clinical care and crisis intervention.
  • Contribute to the development and implementation of policies and procedures related to clinical care and crisis intervention.

Requirements

  • Qualified Registered Mental Health/Learning Disability Nurse or Social Work qualification.
  • Active registration on the appropriate professional register (NMC, etc.).
  • Evidence of specialized continued professional training (degree level) in clinical practice.
  • Relevant experience of working with adults and/or children/young people with a learning disability and/or autism.
  • Relevant experience of working with adults with mental health needs, or clear evidence of substantial experience of working with individuals in crisis and skills that are transferable to both community/in-patient settings.
