Key Responsibilities:

Your Skills and Qualifications:
• Relevant qualifications in social care or a related field (e.g., degree in Social Work, Healthcare, or a related discipline).
• Proven experience in care coordination or a related field, demonstrating your ability to collaborate effectively with diverse stakeholders.
• Excellent communication and interpersonal skills, enabling effective collaboration with colleagues, clients, and external stakeholders.
• Strong problem-solving abilities, adaptability, and flexibility, allowing you to navigate complex situations and prioritize tasks efficiently.
