About the Role

We are seeking a highly skilled and motivated Support Worker to join our team at Bedfordshire Hospitals NHS Foundation Trust. As an Independence Support Worker, you will play a pivotal role in promoting reablement and well-being across all medical wards.

Key Responsibilities
  • Assist qualified therapists and nurses in the assessment, treatment, maintenance, and rehabilitation of patients admitted to the ward, maximizing their potential and reducing the risk of deconditioning.
  • Carry out treatment programmes and facilitate independence with personal activities of daily living (ADLs) on the ward, simulating a typical functional day for the patient.
  • Identify and extract relevant information accurately from patients' medical notes, admission paperwork, and patient and carer information to ensure accuracy of recent functional status.
  • Inform patients and families about the ISW role, gain valid informed consent, and promote reablement and rehabilitation.
  • Monitor and analyze patients' progress and behaviors during and following sessions, recording, understanding, and interpreting clinical observations.
  • Make recommendations to ward staff about physical and functional ability of patients under your care, following discussion with a therapist.
  • Conduct formal and direct functional assessments with therapy assistants or students, analyzing and reporting findings to the relevant therapist.
  • Maintain up-to-date, comprehensive, and accurate patient treatment/care records.
  • Conduct clinical care and treatment with respect to individual patients' dignity, privacy, and cultural needs at all times.
  • Respect the diversity, values, cultural, and religious beliefs of patients/families and contribute to the provision of a service sensitive to those needs.
  • Have a sound working knowledge of the multidisciplinary team (MDT) and the availability of follow-up services.
Requirements
  • Education to GCSE standard or equivalent (including grade C/4 or more in English).
  • Experience and challenges of establishing working relationships with people.
  • Ability to work in a team, independently, and adapt to the needs of a fast-paced ward setting.
  • Ability to communicate using alternative methods where there may be barriers to understanding.
  • Importance of maintaining confidentiality within a healthcare environment.
  • Demonstrate an understanding of reablement and the role of the ISW.
