Job Summary:

We are seeking a highly skilled and motivated Independence Support Worker - Clinical Facilitator to join our team at Bedfordshire Hospitals NHS Foundation Trust. As an Independence Support Worker - Clinical Facilitator, you will play a vital role in supporting patients to achieve their independence and maximize their potential.

Key Responsibilities:

  • Clinical Facilitation: Assist qualified therapists and nurses in the assessment, treatment, maintenance, and rehabilitation of patients admitted to the ward.
  • Rehabilitation Support: Carry out treatment programmes and facilitate independence with personal activities of daily living (ADLs) on the ward.
  • Communication and Teamwork: Communicate effectively with patients, families, and healthcare professionals to ensure seamless care delivery.
  • Record Keeping and Data Management: Maintain accurate and up-to-date patient records, including treatment plans, progress notes, and statistics.
  • Professional Development: Participate in ongoing training and professional development to enhance skills and knowledge in clinical facilitation and rehabilitation support.

Requirements:

  • Education and Qualifications: GCSE standard or equivalent (to include grade C/4 or more in English)
  • Experience: Experience in healthcare or social care settings, with a focus on rehabilitation and patient support
  • Skills: Ability to work independently and as part of a team, excellent communication and interpersonal skills, and a commitment to patient-centered care
