Specialist Dementia Practitioner

You will be a member of the multi-disciplinary memory assessment service team and as an independent specialist practitioner, you will ensure the effective assessment, planning, consultation and formulation towards diagnosis.
To have a specialist knowledge and understanding of strategies and interventions in the treatment of dementia. To be proficient in referring to other agencies within the service network.
Key Responsibilities:
- Conduct comprehensive assessments of individuals with dementia
- Develop and implement individualized care plans
- Collaborate with multidisciplinary teams to provide holistic care
Working Arrangements:
This is a full-time position with regular working hours. The successful candidate will be required to work flexibly to meet the needs of the service.
