Patient Care Navigator

The Patient Care Navigator is a vital member of the healthcare team, responsible for ensuring that patients receive seamless and coordinated care throughout their hospital stay. As a skilled navigator, you will facilitate smooth transitions between departments, promote patient engagement and empowerment, and optimize health outcomes.
Main Responsibilities:- Patient Navigation: Collaborate with patients, families, and healthcare teams to navigate the complex healthcare system, address concerns, and resolve issues.
- Transition Management: Facilitate transfers between departments, units, and levels of care, ensuring continuity of care and minimizing disruptions.
- Patient Education: Provide personalized education and support to patients and their families, empowering them to take an active role in their healthcare.
- Qualifications: Current RN licensure in Massachusetts, BLS certification, and ACLS certification (if applicable)
- Experience: 1-2 years of experience in medical-surgical or critical care nursing, preferably with experience in navigation or case management
- Skills: Excellent communication and interpersonal skills, ability to think critically and problem-solve, and commitment to delivering patient-centered care
Estimated salary: $75,000 - $95,000 per year.
