About the Opportunity:
The Community Heart Failure Nurse Specialist will be part of a multidisciplinary team, working closely with primary and secondary care services to deliver high-quality patient care. The ideal candidate will possess excellent communication and interpersonal skills, enabling them to build strong relationships with patients, families, and healthcare professionals.
Main Duties:
- Manage complex heart failure patients in the community, ensuring seamless care coordination between services.
- Develop and implement evidence-based interventions to improve patient outcomes, focusing on prevention and early intervention strategies.
- Maintain up-to-date knowledge and skills in Long-term Condition issues, including NICE Clinical Guidelines and local CCG Commissioning Framework guidelines.
Essential Qualifications:
- Postgraduate degree or equivalent in a relevant field.
- At least 5 years of clinical experience in Heart Failure, with 2 years at Band 6 level.
- Non-Medical Prescriber qualification or willingness to work towards it.
