Job Description

We are seeking a Mental Health Care Coordinator to join our team in South Kent Coast, Thanet area. The successful candidate will be responsible for assessing, planning, implementing, and evaluating care for patients allocated to them.

The role involves delivering a flexible and responsive service, responding swiftly to urgent referrals, and recognizing racial, cultural, sexual preference, and addressing health inequalities.

Key responsibilities include ensuring that care plans are reviewed and updated, undertaking risk assessment/management, and disseminating information to team members and professionals involved.

The ideal candidate will have experience working in a community team and hold a diploma or degree qualification as a Registered Mental Health Nurse or Registered Occupational Therapist.
